Data Privacy Notice and Consent Form for Students

Eugenio Cabezas National High School (ECNHS) is required by law to process your personal and sensitive personal information in order to safeguard academic freedom, uphold your right to quality education, and protect your right to data privacy. In conformity with Republic Act No. 10173 or the Data Privacy Act of 2012, its Implementing Rules and Regulations, and other relevant laws, this Data Privacy Notice can be accessed at https://www.officialgazette.gov.ph/2012/08/15/republic-act-no-10173/

Personal Information Collected and Stored

The School collects, uses and keeps your personal information, which include but are not limited to the following:

Name, Age, Date of Birth
Address and Contact Details
Photograph or Image
Family Profile (Names of parents/guardians, their address and contact details, etc.)
Grade Level
Study Documents and Geographic Information (Address, etc.)

Use of Personal Information

The following information will be used for the following purposes, which you have consented to or other authorized representatives during your application for admission and in the course of determining your eligibility to enroll in ECNHS:

  • To document consent for the processing of personal data in the context of determining your eligibility to enroll in ECNHS
  • For the purposes such as but not limited to:
    • provision of psychological and emotional assessment as needed
    • provision of various student support services within the school (i.e. guidance services, clinic services, etc.)

Sensitive Personal Information Collected and Stored

The School also processes sensitive personal information under the DPA. These include, but are not limited to: race, ethnicity, civil status, health information, government-issued ID and relevant financial information.

Access and Disclosure

Access to the data is restricted and given only to predetermined authorized personnel in relation to their specific function which requires them to access or process student or employee information.

Disclosure

In comply with its legal and regulatory duties, the School submits information to relevant government agencies such as the Department of Education (DepEd). As a rule, ECNHS will only disclose or share your personal information with your written consent, and will disclose or share such information only when required or allowed by applicable laws.

Security

Your personal data is stored in physical repositories and secured databases managed by the School's Registrar and ICT Office. The School has appropriate physical, technical and organizational safeguards to ensure the confidentiality of your information. These measures will be reviewed over time and upgraded in line with technological advancements.

By continuing, you confirm that you have read the ECNHS Data Privacy Notice and Consent Form, understand its contents and consent to the processing of your personal data.
